Reporting an Injury

Report work-related injuries and illnesses, regardless of whether medical treatment is required, to the Office of Human Resources within three (3) days or seventy-two (72) hours of the incident. Prompt reporting ensures that the injured person receives the medical treatment and benefits entitled to them.

To report an injury or illness, the EMPLOYEE should:

  1. Notify your supervisor of the injury or exposure.
  2. Complete the injury report form here.
  3. Print, sign, and email the form to workerscompensation@morgan.edu
  4. Include your full name in the subject line of the email.

The SUPERVISOR should:

  1. Complete the supervisor report of injury form here.
  2. Print, sign, and email the form to workerscompensation@morgan.edu.
  3. Include the injured employee's full name in the email's subject line.
  4. If you know of any witnesses, forward them this form (witness instructions below).

Any WITNESSES should:

  1. Complete the witness report of injury form here.
  2. Print, sign, and email the form to workerscompensation@morgan.edu.
  3. Include the injured employee's full name in the email's subject line.

After Reporting an Injury

Upon receipt of an employee's report of an injury, the Office of Human Resources (OHR) will report the incident to the state Injured Workers Insurance Fund for processing. The OHR will provide the injured employee with a claim number, further instructions for seeking medical treatment, and timesheet procedures, if necessary.

The OHR will also communicate with the supervisor regarding any potential restriction of duties, workers' comp leave of absence, including timesheet completion instructions, and return to work expectations.
