(MANDATORY) 30-Hour Rule Waiver Form

ATTENTION: Your "Undergraduate Student Request to Pursue a Course Off-Campus" form WILL BE DECLINED if this form is not attached to your "Undergraduate Student Request to Pursue a Course Off-Campus" form with approved signatures OR if fields in this form are incomplete or missing.

1) This form acknowledges that you understand the protocols and circumstances regarding the Morgan State University 30-Hour Rule Waiver.

2) This form MUST be signed with approved signatures and submitted along with the official off-campus college or university course description (downloaded from the school's website).

3) The TWO FORMS MUST be attached to the "Undergraduate Student Request to Pursue a Course Off-Campus" form.

This form is for students who wish to request to take a course at another college or institution and have the credits applied to an equivalent course at Morgan State University.

Grade Appeal
This form initates the process for a department grade appeal.
