• PF10 Contractual Personnel Request Form

The PF10 Contractual Personnel Request Form is exclusively for revisions and continuation requests. For first-time and repeat employees, the EPAF system should be used and is available on WebSIS. If you need assistance with submitting an EPAF contract, please review the ORA e-training module on Contractual Salaries here.

Please note that the Check Issue Date MUST be at least 30 days out from the date of the memo's submission.

While these are the forms that are required for grant-funded travel, the University Travel website has further details on how to complete the form correctly, what to include and/or attach, and how best to ensure timely return of a TL number.

• Grant-Funded GRADUATE ONLY Tuition and Fees Request Memo

Please note that for GRADUATE students, grant-funded health insurance cannot be requested through a memo. Grant-funded health insurance can be reimbursed to the graduate student through a requisition.
