Required Health Center Documents

To keep the MSU community healthy, every student is required to comply with Morgan State University Mandatory Health documentation requirements. Morgan State University requires students to submit their Health Insurance card, complete the Health Entrance Certificate, and submit their Immunization records. All Health Documentation must be submitted upon enrollment.

Additionally, All Student Athletes are required to submit a Sickle Cell Test with their Immunization Records. If you would like to receive a Sickle Cell Test at the University Health Center, please follow the instructions on the Services and Fees web page, to ensure you pay for the Sickle Cell Test, before contacting the University Health Center.

How To Submit Your Health Entrance Certificate

All incoming Morgan State University Undergraduate and Graduate students are required to complete the Mandatory Health Entrance Certificate prior to attending the University. The completed and signed Health Entrance Certificate must be returned to the University Health Center, or your Morgan State University registration will be denied. Please click on the link below to access the Health Entrance Certificate.

How To Submit Your Health Entrance Certificate:

  1. Complete & Sign your Health Entrance Certificate. Please complete and sign the Health Entrance Certificate.
  2. Submit your Health Entrance Certificate. Please email a copy of your Health Entrance Certificate. to healthcenter@morgan.edu. Please include your full name, student identification number, and date of birth in the email. 
  3. Submission Deadline.The submission of your Health Entrance Certificate is due by August 1st for Fall Enrollment and January 1st for Spring Enrollment.

NOTE: Please complete ALL sections of the Health Entrance Certificate. If you fail to complete ALL sections, we will return the document. We will request a Treatment Plan from your Physician for all sections marked YES in the Personal History Section of the Health Entrance Certificate.

The University Health Center staff reviews all documents submitted for verification. Please allow 2-3 business days for the Health Center staff to review your documents and update your chart. How To Submit Your Immunization Records

All incoming Morgan State University Undergraduate and Graduate students are required to submit their Immunization Records prior to attending the University. The submission of your Immunization Requirements and TB Test must be submitted prior to the deadline below or your Morgan State University registration will be denied. 

Morgan State University Student Health Center requires proof of the following Immunizations and TB Test:

  • Immunization for MMR #1 and MMR #2 (Measles, Mumps, Rubella)
  • Hepatitis B (1,2,3)
  • Tdap (Diphtheria, Tetanus, and A cellular pertussis booster vaccine within 10 years)
  • Meningitis vaccine
  • Tuberculosis Test (TB/PPD)- ONLY REQUIRED FOR INTERNATIONAL STUDENTS. Test must be administered within the past year

The Immunization Records are mandatory and must be documented with dates administered by the health care provider or lab. 

Morgan State University Student Health Center recommends but does not require proof of the following Immunizations:

  • COVID-19 vaccine

How To Submit Your Immunization Records:

  1. Collect your Immunization Records. Please collect a copy of your Immunization Records and TB Test Results.
  2. Submit your Immunization Records. Please log into myhealthportal.morgan.edu with your MSU student login credentials. Once you have logged in, please enter your Date of Birth (MM/DD/YYYY) and click Proceed. Click on Immunization Upload and add your full Immunization Record and click Save. 
  3. Submission Deadline.The submission of your Immunization Records is Sunday, June 21, 2024.

How To Submit Your Health Insurance Card

The submission of your Health Insurance Card supports the processing of external labs (urinalysis and blood work) during your schedule appointment in the University Health Center. Your Health Insurance carrier determines the total amount you will be directly billed for external laboratory services. THIS IS NOT THE STUDENT HEALTH INSURANCE WAIVER.

How To Submit Your Health Insurance Card:

  1. Collect your Health Insurance Card.
  2. Submit your Health Insurance Card. Please log into myhealthportal.morgan.edu with your MSU student login credentials. Once you have logged in, please enter your Date of Birth (MM/DD/YYYY) and click Proceed. Click on Insurance Card and then click on Add New Card . Upload a front and back copy of your Insurance Card and click Save.
